MISSION: AGAPE exists to strengthen families and communities by providing compassionate, community-driven support that addresses food insecurity, basic needs, financial stability, and overall wellness. Through relationship-based care, strategic partnerships, and coordinated Social Determinants of Health initiatives, MISSION: AGAPE helps individuals move from crisis toward long-term stability, dignity, and opportunity.

BRIDGES TO CARE

The Bridges To Care program is a community-based support model designed to bridge the gap between residents, healthcare systems, and essential resources across the Mon Valley. Through trusted neighborhood-based Community HEALrs, the program provides personalized concierge service with relationship-driven guidance that helps individuals and families navigate challenges related to food insecurity, transportation, healthcare access, financial instability, housing, and workforce readiness. Rather than offering one-time referrals, HEALrs walk alongside participants through a concierge-style process, conducting needs assessments, coordinating services, connecting residents to clinical and social supports, and providing ongoing follow-up to ensure barriers are actually resolved.

LAUNCH SWPA

Launch SWPA’s Workforce Development and Entrepreneurial Starter Program is designed to help individuals across Southwestern Pennsylvania build practical pathways toward employment, self-sufficiency, and small business growth. The program combines workforce readiness training, soft skills development, financial literacy, digital competency, mentorship, and entrepreneurial education into an accessible community-based model focused on historically underserved communities. Participants receive hands-on support with resume building, interview preparation, career exploration, business startup fundamentals, and navigating barriers such as transportation, technology access, and economic instability. Through partnerships with community organizations, educational institutions, employers, and local entrepreneurs.
